Together, these team members create an individualized care plan for each resident designed to meet their unique needs. Care plans are reviewed every three months by the Interdisciplinary Care Team. In addition, Christian Health Rehabilitation services works closely with the IDC to improve residents’ quality of life through programs such as Balance and Fall Prevention, Seating and Positioning, Contracture Management, and more. Additional specialized consultants include dentists, podiatrists, and psychiatric-service professionals.
When someone you love has Alzheimer’s disease or another form of dementia, caring for them can be challenging. Our staff members are specially trained in dementia care and offer watchful, compassionate attention through a resident wander-prevention system. In addition, a structured, specialized calendar of therapeutic activities is created to meet the specific needs and abilities of residents.
Heritage Manor is Medicare- and Medicaid-certified with accommodations for 134 patients in private and semi-private rooms. Large windows in every room provide natural light and beautiful views of the Ramapo Mountains, the woods and wildlife. Each room is pleasantly furnished with a bed, dresser, night table, over-bed lights for reading, a comfortable high-back chair, and flat-screen television. We also encourage residents and their families to personalize the rooms to their own taste with pictures and favorite accessories to create a home-like atmosphere.

It almost goes without saying, but food is an essential part of everyday life. Food nourishes and strengthens us, engages the senses and provides opportunities for socialization as friends gather for meals. We serve three healthy, delicious meals each day, plus snacks. Our menus include a wide variety of food choices, with both a restaurant-style, dining program and an assisted-dining program.
