Bolger Short-Term Rehab

A Multidisciplinary Approach to Care

The David F. Bolger Post-Acute Care Unit (PACU) on Christian Health’s campus in Bergen County, NJ, provides short-term rehabilitation for people recovering from an illness, injury, or surgery. Within the compassionate, caring environment for which Christian Health is known, patients receiving Bolger Short-term Rehab benefit from the skills and knowledge of a full-time, on-site Interdisciplinary Care (IDC) Team that includes:

  • Physicians
  • Physiatrist
  • Registered nurses
  • Licensed practical nurses
  • Certified nursing assistants
  • Physical therapists
  • Occupational therapists
  • Speech therapists
  • Social workers
  • Discharge planners
  • Activities staff
  • Chaplains

Although Christian Health employs specialized physicians who will direct your post-acute care, you may continue to receive care, if necessary, from your personal physician and any outside medical services that are not available in the PACU, such as dialysis. Our Coordination of Physician Services office will schedule appointments and coordinate your transportation.

Rehabilitation Services: Physical, Occupational, and Speech Therapy

Rehabilitation (rehab) services are a key component of the time you will spend here. Rehab helps you work toward restoring the function that has been compromised by surgery, illness, injury, or disease.

Individualized rehabilitation plans are created by the Rehabilitation Services staff in collaboration with other professionals on your IDC Team. Your overall health, age, and physical limitations are taken into consideration when developing a plan, as are physiological changes and decreases in exercise tolerance, maximum heart rate, muscle strength, joint mobility, peripheral blood flow, vision, hearing, motor speed skills, coordination, and balance. Most rehabilitation plans include a personalized combination of physical therapy and occupational therapy and, when appropriate, speech therapy.

Once a plan has been established, a physiatrist and a team of skilled, licensed physical, occupational, and speech therapists will provide intensive therapy in an effort to help you return home safely and as healthy as you were before—or better.

Patient and Family Education

Patient and family education is a significant component of Bolger Short-term Rehab. Education focuses on your condition, medications, necessary precautions, home-care possibilities, and post-discharge medical appointments.

You Are an Important Part of the Care Team

A large part of the multidisciplinary-care plan involves continual input from the patient and his or her family, who are also members of the care team. The team meets formally seven to 14 days after admission, depending on the complexity of the case. Because stays are typically short, discharge planning begins at the first IDC meeting. In general, subsequent meetings take place on a weekly basis throughout a patient’s stay.

Dining Services

Delicious, healthy, restaurant-style meals are served in our attractive dining room. Meal times give patients an opportunity to socialize and interact with others who are facing similar challenges.

Achieving Goals and Returning Home

Your progress in rehabilitation is a major driving factor in determining when you can be discharged, and it will be monitored and measured on a daily basis. Another crucial factor is your clinical needs—how you are adjusting to new medications or how well you are learning to use necessary medical equipment, such as a glucose monitor.

A patient is discharged from the PACU when he or she can return home safely and at his or her pre-hospital health status. When necessary, post-discharge services such as visiting health services or Meals On Wheels are arranged.

Although the average length-of-stay is 21 days, Medicare patients may remain in Bolger Short-term Rehab until they are discharged from the Medicare program.
