Community Health Needs Assessment

In collaboration with other area healthcare providers, the Community Health Improvement Partnership of Bergen County, and the Bergen County Public Health Department, we are conducting a survey to assess community health needs. The outcome of the assessment is to identify gaps and help inform what additional programs and services are needed for optimal health in our community.
