Christian Health's Home Care Options is now an official screening site for the Alzheimer's Foundation of America (AFA).

Whether you've noticed changes in your memory or simply want to check in on your cognitive health, a memory screening can help identify potential concerns and determine whether further evaluation may be helpful.*

What to Expect

• Confidential, face-to-face assessment conducted by a certified screener
• Personal review of your results, including recommendations for follow-up if needed
• A copy of your results to share with your healthcare provider
• Approximately 15 minutes per session
